Pathophysiologie der autoinflammatorischen Dermatosen

Zusammenfassung

Autoinflammation führt zur Entzündung, die klinisch meist ohne ersichtlichen Grund auftritt und so schwer sein kann, dass Organpathologie mit relevanter Gewebeschädigung auftritt. Das Inflammasom gilt als Motor der Autoinflammation. Obschon IL(Interleukin)-1β und das Inflammasom als sein kritischer Regulator sehr wichtig sind bei der Autoinflammation, sprechen nicht alle Patienten auf die Hemmung dieses Signalwegs an. Mehrere autoinflammatorische Krankheiten wurden mit Mutationen in Proteasom-Immunoproteasom-Komponenten assoziiert. Die durch hochrelevante Genvarianten bedingten autoinflammatorischen Erkrankungen sind meist erblich bedingt. Diese Krankheiten beginnen meist im pädiatrischen Lebensalter, aber nicht immer. Die nächsten Jahre werden zeigen, ob wir entzündliche Dermatosen zusätzlich zur Hemmung der adaptiven Immunität vermehrt mit Bremsung des angeborenen Immunsystems behandeln werden.
