Immunologische Therapieansätze

Zusammenfassung

Mithilfe vieler technischer Erneuerungen im IVF-Labor und durch die Optimierung der Stimulationsbehandlung ist es in den letzten Jahren gelungen, die Schwangerschaftsrate nach einer IVF/ICSI-Behandlung deutlich zu steigern. Dennoch bleiben etwa 20–30 % der Paare kinderlos. Einen möglichen Faktor stellen dabei immunologische Störungen dar, welche im Bereich des Endometriums bzw. im peripheren Blut Einfluss auf die Implantation nehmen. Die Vielzahl an immunkompetenten Gegenspielern umfasst sowohl periphere und uterine natürliche Killerzellen als auch Zytokine, T- und B-Zellen sowie weitere (Auto-)Immunzellen. Bislang ist es nicht möglich, eine spezifische immunologische Diagnostik einer gezielten Therapie zuzuordnen, auch wenn eine Vielzahl an immunmodulatorischen Medikamenten zur Verfügung steht. In einzelnen Studien konnte teilweise ein Benefit hinsichtlich der Lebendgeburtenrate beim Einsatz von Immuntherapeutika in der Reproduktionsmedizin gezeigt werden. Bislang sind die Fallzahlen jedoch noch relativ klein, weshalb diese Therapieformen v. a. in klinischen Studien angewandt werden sollten. Dieser Übersichtsartikel fasst die aktuelle Datenlage der in der Reproduktionsmedizin verwendeten immunmodulatorischen Therapien wie TNF-Inhibitoren, Granulozyten-Kolonie-stimulierender Faktor (G-CSF), Immunglobuline und Lipidemulsionen zusammen.
