Endoscopic resection techniques were largely restricted to endoscopic polypectomy and endoscopic mucosal resection (EMR) until the early 1990s. However, these procedures had limitations with respect to size of lesion that could have been resected, low en bloc resection rates and higher risk of recurrence [
1]. The concept of third-space endoscopy started in Japan in the 1990s with endoscopic sub-mucosal dissection (ESD), which visualized and required sub-mucosal space dissection. It involves complete removal of superficial lesions through dissection of the sub-mucosal layer. ESD was initially developed to overcome the limitations of EMR to achieve en bloc resection of superficial lesions of more than 20 mm [
2]. Gotoda and colleagues reported the first successful ESD for early gastric cancer in 1999, followed by rectal ESD in two cases in the same year [
