Aktuelle Konzepte zur funktionellen Dyspepsie

Zusammenfassung

Der Terminus „Dyspepsie“ beschreibt chronische oder intermittierende Beschwerden des oberen Gastrointestinaltrakts, die durch epigastrische Schmerzen, Appetitlosigkeit, Völlegefühl, Aufstoßen, Sodbrennen, Übelkeit oder Erbrechen charakterisiert sind [3]. Die Prävalenz dyspeptischer Symptome liegt in der Normalbevölkerung bei etwa 15–20%. Die meisten dieser Patienten leiden unter einer funktionellen Dyspepsie (FD), während organische Ursachen wie peptische Ulzera, Ösophagitis bei gastroösophagealem Reflux (GERD) sowie gastrointestinale Neoplasien einen wesentlich geringeren Anteil ausmachen [12]. Die Ätiologie der funktionellen Dyspepsie ist bislang nur partiell verstanden. Gemäß den Rome-III-Kriterien ist die FD durch distinkte epigastrische Beschwerden ohne Hinweis auf strukturelle oder biochemische Anomalien definiert [35]. Aktuelle Hypothesen gehen jedoch von Alterationen der Motilität und einer viszeralen Hypersensitivität als zugrunde liegenden Pathomechanismen aus. In der symptomatischen Pharmakotherapie kommen daher insbesondere Substanzen zur Modulation der Motoraktivität und der viszeralen Nozizeption zur Anwendung.
