Aktuelle Seroprävalenz, Impfstatus und prädiktiver Wert der Leberenzyme für Hepatitis B bei Flüchtlingen in Deutschland

Zusammenfassung

Hintergrund

Über die Seroprävalenz, den Impfstatus und die Sensitivität der Leberenzymbestimmungen als Hinweis auf eine Hepatitis B bei Flüchtlingen, die im Rahmen der aktuellen Flüchtlingskrise nach Deutschland migrieren, existieren nur Schätzungen.

Ziel der Arbeit

Dokumentation der Seroprävalenz und des Impfstatus von Hepatitis B bei Flüchtlingen in Norddeutschland im Jahr 2015.

Methoden

In einer Querschnittsuntersuchung erfolgte in sechs Erstaufnahmeeinrichtungen im August 2015 bei 793 Patienten aller Altersgruppen eine Bestimmung der serologischen Marker einer Hepatitis-B-Virusinfektion (HBsAg, anti-HBc). Darüber hinaus wurden ALT, AST, Bilirubin, γGT und alkalische Phosphatase bestimmt. Bei 258 Patienten wurden zusätzlich anti-HBs Antikörper bestimmt.

Ergebnisse

76,7 % der Untersuchten waren Männer, das mittlere Alter lag bei 28,8 ± 11,4 Jahren. 7,8 % waren Kinder unter 18 Jahren. Die Gesamtprävalenz von HBsAg betrug 2,3 %, die von anti-HBc-Antikörpern 14,3 % (2,5 % bzw. 14,5 % bei Männern und 1,2 % bzw. 13,5 % bei Frauen). Die höchste Prävalenz zeigte sich für das HBsAg zwischen dem 35. und 49. Lebensjahr mit 3,1 % und für das anti-HBc bei über 50-jährigen Flüchtlingen mit 38 %. 62 % der Untersuchten wiesen keine Immunität gegen Hepatitis B auf. 18,6 % der Gesamtpopulation waren geimpft, während bei Kindern unter 15 Jahren (n = 12) 50 % einen Impfschutz aufwiesen. Der positive prädiktive Wert von erhöhter AST und ALT für das Vorliegen eines HBsAg war 0 bzw. 0,016. Nur zwei Patienten mit positivem HBsAg wiesen erhöhte Transaminasen auf.

Diskussion

Diese Studie zeigte eine höhere Prävalenz von HBsAg und anti-HBc bei Flüchtlingen in Deutschland im Vergleich zur deutschen Normalbevölkerung. Leberenzyme sind für das Screening auf Hepatitis B in dieser Population wenig hilfreich.
