Einsatz von Akupunktur bei Posttonsillektomieschmerz

Eine prospektive, randomisierte und kontrollierte Doppelblindstudie

Zusammenfassung

Hintergrund

Der postoperative Schluckschmerz ist eine der unangenehmsten Nachwirkungen nach Tonsillektomie. In den letzten Jahren ist die Nachfrage nach Alternativen zur medikamentösen Schmerztherapie immer weiter gestiegen, jedoch wurde auf diesem Gebiet bisher wenig geforscht.

Material und Methoden

Randomisiert wurden 46 Patienten in eine Verum-Akupunktur-, eine Kontroll-Akupunktur- und eine Medikamentengruppe eingeteilt. Alle Patienten erhielten nichtsteroidale Antirheumatika (NSAR). Die Verumgruppe erhielt eine Stunde nach Medikamenteneinnahme zusätzlich eine Akupunktur entsprechend den Regeln der Foraminologie (S34, S44 und PC5). Die Kontrollgruppe wurde an unspezifischen Punkten in der Medioaxillarlinie akupunktiert. Durchgeführt wurde die Akupunktur von einem verblindeten Akupunkteur, der im Vorfeld ausschließlich diese beiden Techniken erlernte. Die Patienten wurden aufgefordert, ihr Schmerzempfinden vor und in Zeitabständen von 20 min, 1 h, 2 h und 3 h nach der Medikamenteneinnahme bzw. der Akupunktur auf einer Visuellen Analogskala (VAS) abzuschätzen.

Ergebnisse

Die schmerzlindernde Wirkung der Akupunktur war in der Verum-Akupunkturgruppe bis 3 Stunden nach der Akupunktur signifikant (p < 0,05). Die analgetische Wirkung in der Kontroll-Akupunkturgruppe war bis eine Stunde nach Akupunktur signifikant (p < 0,05). Bezogen auf den Zeitpunkt vor der Akupunktur waren die Differenzen beider Akupunkturgruppen im Vergleich zur Medikamentengruppe signifikant (p < 0,01) über den gesamten Zeitraum.

Schlussfolgerung

Die Akupunktur ist eine effektive Ergänzungstherapie zu NSAR in der Behandlung des Posttonsillektomieschmerzes. Vor allem Patienten mit Allergien, Medikamentenunverträglichkeiten oder vermindertem Ansprechen auf die übliche Medikation könnten von einer Akupunktur profitieren.
