Akute, bilaterale Visusminderung bei einem Patienten mit IgA-Glomerulonephritis

Zusammenfassung

Wir berichten über einen 30-jährigen Patienten mit akuter beidseitiger Visusminderung. Funduskopisch imponierten beidseitig retinale Blutungen, Cotton-Wool-Herde, Makulaödem und Papillenhyperämie. Die Veränderungen waren vereinbar mit einer hypertensiven Retinopathie Stadium IV. Die notfallmäßige internistische Vorstellung ergab eine bisher unbekannte IgA-Glomerulonephritis mit terminaler Niereninsuffizienz und sekundärer Blutdruckentgleisung. Ein akuter Fundus hypertonicus kann als Erstsymptom einer Niereninsuffizienz auftreten und bedarf einer notfallmäßigen internistischen Abklärung.
