Management der schweren Dehydratation beim Kind

Zusammenfassung

Bei der Dehydratation eines Kindes ist auf die Schwere der Exsikkose und auf Zeichen eines Volumenmangelschocks zu achten, da sich daraus die weiteren Maßnahmen ableiten. Bei milder bis moderater Dehydratation kann die Therapie in der Regel mit oraler Rehydratationslösung erfolgen. Bei schwerer Exsikkose ist eine intravenöse Volumensubstitution unter Beachtung der Elektrolyte notwendig. Für den initialen Volumenbolus und den weiteren Flüssigkeitsausgleich eignen sich glukosefreie balancierte Vollelektrolytlösungen. Insbesondere eine Hypo- oder Hypernatriämie muss unter engmaschigen Kontrollen ausgeglichen werden, um neurologische Komplikationen zu vermeiden, je schwerer die Ausprägung, desto vorsichtiger. Bei jeder Form der Dehydratation ist eine regelmäßige Evaluation des Kindes wichtig – mindestens nach jeder durchgeführten Maßnahme. Dabei werden Bewusstsein, Hautturgor, Kapillarfüllungszeit, Herzfrequenz, Blutdruck, Diurese und Körpergewicht sowie Elektrolyte, Blutzucker und Laktatspiegel beurteilt.
