Zusammenfassung

In den letzten Jahren hat die Diagnostik von Lungenarterienembolien (LAE) an Bedeutung gewonnen, da nur bei etwa 15–25 % der Verdachtsfälle letztendlich eine Bestätigung erfolgt. Trotz technologischer Fortschritte bleiben radiologische Verfahren aufgrund von Strahlen- und Kontrastmittelbelastung problematisch. Die klinischen Scores spielen eine entscheidende Rolle hinsichtlich der Prätestwahrscheinlichkeit einer LAE. Hochwahrscheinlichkeitssituationen erfordern spezifische Maßnahmen, während negative D‑Dimere Überdiagnostik vermeiden können. Die computertomographische Pulmonalisangiographie (CTPA) bleibt der Goldstandard mit hoher Sensitivität und Spezifität. Die Therapie erfordert ein interdisziplinäres Team („pulmonary embolism response team“, PERT). Antikoagulation ist bei stabilen Patienten eine Option, während bei instabilen oder erfolglosen Verläufen eine Thrombolyse oder interventionelle Verfahren in Betracht gezogen werden können. Nebenwirkungen, insbesondere die Blutungsgefahr, sind bei beiden Therapiearten zu berücksichtigen.
