Standards der kurativen Systemtherapie der akuten myeloischen Leukämie

Zusammenfassung

Hintergrund

Die akute myeloische Leukämie (AML) ist eine biologisch heterogene Erkrankung mit Implikationen sowohl für Prognose als auch Therapie.

Ziel

Seit 2017 sind verschiedene neue Medikamente zur zielgerichteten Therapie der AML zugelassen worden. Diese Arbeit gibt eine Übersicht über die aktuellen Behandlungsempfehlungen, wenn ein kurativerer Therapieansatz verfolgt werden kann.

Material und Methoden

Grundlage für die Zusammenfassung sind die aktuellen Onkopedia-Leitlinien zur AML.

Ergebnisse und Schlussfolgerung

Die kurative Therapie der AML besteht aus Induktions‑, Postremissions- und ggf. Erhaltungstherapie. Je nach Anamnese und genetischem Status besteht die Induktionstherapie aus dem klassischen „7 + 3-Schema“ oder CPX-351. Zusätzlich sollte das „7 + 3-Schema“ je nach vorhandenen Mutationen und Immunphänotyp ggf. um Midostaurin bzw. Gemtuzumab-Ozogamicin (GO) ergänzt werden. Die Postremissionstherapie besteht je nach Risikoprofil aus Hochdosischemotherapie (± Midostaurin bzw. ± GO) oder allogener Stammzelltransplantation. Für einige Patienten kann im Anschluss noch eine Erhaltungstherapie indiziert sein. Im Fall eines Rezidivs kann eine chemotherapiebasierte Reinduktion erfolgen oder bei Patienten mit FLT3-Mutation eine zielgerichtete Therapie mit Gilteritinib. Anschließend sollte - wenn möglich - eine allogene Stammzelltransplantation durchgeführt werden.
Die Therapie der AML sollte an einem hämatologisch-onkologischen Zentrum und möglichst im Rahmen einer klinischen Therapiestudie erfolgen.

In der MONARCH-3-Studie erhielten Patientinnen mit fortgeschrittenem HR+, HER2- Brustkrebs Abemaciclib [1,a] in Kombination mit nicht-steroidalem Aromatasehemmer (nsAI).
