53. Allergenkarenz und Klimatherapie

Zusammenfassung

Der Kontakt zu Nahrungsmittel- oder Inhalationsallergenen kann bei entsprechender Prädisposition zur Sensibilisierung und zu klinischen Reaktionen im Sinn einer Allergie führen. Eine vollständige Karenz ist nicht möglich, auch gibt es keine klaren Angaben zu Schwellenwerten, die eine Sensibilisierung unwahrscheinlich machen würden. Hinsichtlich der Vermeidung von Nahrungsmitteln wie Kuhmilch im Säuglingsalter gibt es kaum Beweise, dass eine Nahrungsmittelallergie verhindert wird. Lediglich die Manifestation des frühen atopischen Ekzems scheint unter Hydrolysaternährung reduziert zu werden. Ergebnisse von Studien, die die frühe Beikosteinführung und damit nutritive Allergenexposition kontrolliert untersuchen, werden mit Spannung erwartet. In Hinblick auf die Innenraumallergenexposition gibt es klare Unterschiede je nach geografisch bedingter Variabilität des Allergenspektrums. In Skandinavien zeigen sich bei niedriger Hausstaubmilbenexposition auch seltener Milbenallergien. Jedoch enttäuschen Primärpräventionsstudien in Regionen mit hoher Milbenallergenexposition (England). Als Tertiärprävention, also Therapie bei manifester Tierhaar- oder Hausstaubmilbenallergie, kann die konsequente Allergenreduktion im häuslichen Milieu das Beschwerdeniveau senken.
