Allergieprävention bei Asthma bronchiale

Sinnvolles Vorgehen

Zusammenfassung

Asthma bronchiale ist eine der häufigsten chronischen Erkrankungen im Kindes- und Jugendalter. Neben den individuellen krankheitsbedingten Beeinträchtigungen ist die gesellschaftliche ökonomische Belastung durch die Behandlung von Asthma bronchiale hoch, sodass dringend präventive Ansätze benötigt werden. Als wirksame primärpräventive Maßnahmen haben sich multifaktorielle Karenzmaßnahmen, v. a. gegen Hausstaubmilben, Passivrauchexposition und Tierepithelien erwiesen. Medikamentöse prophylaktische Behandlungen als sekundäre Prävention konnten nur die klinische Symptomatik beeinflussen, nicht jedoch den natürlichen Asthmaverlauf. Eine spezifische Immuntherapie hingegen erscheint wirksam. Tertiäre präventive Maßnahmen stellen v. a. soweit möglich die Allergenkarenz sowie die adäquate medikamentöse Therapie des geschulten Patienten dar. Innovative Verfahren wie die unspezifische Immunmodulation oder aber die gezielte Hyposensibilisierung in kritischen Lebenszeitfenstern könnten in Zukunft die Palette möglicher Maßnahmen erweitern.
