Allergenspezifische Immuntherapie mit Vorratsmilben — eine vernachlässigte Indikation

Zusammenfassung

Aufgrund der Therapieallergene-Verordnung (TAV) und wegen berechtigter Forderungen nach großen, doppelblinden, placebokontrollierten Studien in der allergenspezifischen Immuntherapie (AIT) sind seltener vorkommende Therapieallergene als Immuntherapeutika aus dem Fokus der Wissenschaft und der Allergologen gerückt. Die AIT mit Hausstaubmilbenextrakt ist gut etabliert, aber auch Allergene von Vorratsmilben können ähnliche allergische Symptome wie die Allergene der Hausstaubmilben hervorrufen. Auch in diesen Fällen ist dann eine kausale Behandlung erforderlich. Besonders in Asien zählen Vorratsmilben zu den wichtigsten inhalativen Allergenen, aber auch in Deutschland sind sie für einige Patienten wichtige Allergieauslöser. Die Kreuzreaktivität zwischen Hausstaub- und Vorratsmilben ist gering, ebenso wie die Kreuzreaktivität zwischen den verschiedenen Vorratsmilbenarten. Vor diesem Hintergrund sollten bei Patienten mit allergischer Rhinitis auch die Vorratsmilben in die Diagnostik miteinbezogen werden, um Fehldiagnosen zu vermeiden. Daher sollten die Hersteller die Verfügbarkeit von Vorratsmilbenextrakten für die Diagnostik und die AIT garantieren können. Klinische Studien, die die Wirksamkeit dieser Extrakte dokumentieren, sind notwendig.
