Zusammenfassung

Einseitige Stimmlippenparesen sind eine häufig Ursache für Heiserkeit. Bei Glottisschlussinsuffizienz stößt die konservative Stimmtherapie oft an ihre Grenzen, sodass ein phonochirurgischer Eingriff indiziert ist. Die Injektionslaryngoplastik bzw. Stimmlippenaugmentation ist ein effektives Verfahren zur Behebung der Insuffizienz. Eine frühe Intervention in den ersten 6 Monaten nach Diagnosestellung wird angestrebt, um eine sich im Laufe der Zeit anbahnende Fehlkompensation abzuwenden. Vor allem Patienten mit einem Glottisspalt von ≤2 mm in der Stroboskopie profitieren von dem Therapieverfahren. Bei entsprechender Patientenselektion wird ein anhaltender Effekt von mehr als 12 Monaten für Hyaluronsäure, Kalziumhydroxylapatit und autologes Fett beschrieben. Aufgrund seltener Komplikationen, z. B. einer allergisch bedingten Schwellung am Injektionsort, werden konsequente laryngoskopische Kontrollen sowie eine Observation für 2 Nächte nach Injektion angeraten. Stimmruhe ist nur für wenige Stunden erforderlich.
