Androgenisierung der älteren Patientin

Besonderheiten in Diagnostik und Therapie

Zusammenfassung

Typische Symptome einer Androgenisierung bei der älteren Patientin sind Hirsutismus und Alopezie. Der Wirkungsgrad der Androgene wird dabei durch den Östrogenmangel, metabolische Einflussfaktoren, aber auch den Rezeptor- und Enzymbesatz am Haarfollikel selbst moduliert. Grundsätzlich ist zwischen einer bereits vorbekannten Hyperandrogenämie beispielsweise bei einem Syndrom polyzystischer Ovarien (PCOS) sowie neu aufgetretenen Beschwerden zu unterscheiden. Davon abhängig stehen eine Anpassung des bisherigen therapeutischen Konzepts infolge altersabhängig veränderter Risiken oder eine vollständige differenzialdiagnostische Abklärung im Vordergrund. Bei rasch progredientem Auftreten einer Androgenisierung mit zusätzlicher Virilisierung muss ein hormonproduzierender Tumor ausgeschlossen werden. Eine entsprechende laboranalytische und bildgebende Diagnostik indiziert in dieser Situation dann die weitere Therapie. Außerhalb dieser Indikation bestehen medikamentöse Optionen in einer antiandrogenen Therapie sowie auch in lokalen Maßnahmen mit nachweislich guter Effektivität zusätzlich zu mechanischen Verfahren.
