Intravenöse Thrombolyse des ischämischen Schlaganfalls – aktueller Stand

Zusammenfassung

Die intravenöse Thrombolysetherapie (IVT) mit Alteplase (rtPA) ist seit ihrer Einführung Ende der 1990er-Jahre wesentlicher Bestandteil der Routinebehandlung von Patienten mit ischämischem Schlaganfall. Einer raschen Behandlung kommt eine wesentliche Bedeutung zu. Bei Patienten mit unklarem Zeitfenster haben sich verschiedene Mismatch-Konzepte etabliert, die von einer IVT noch zu rettendes Hirngewebe identifizieren. Zahlreiche offizielle Kontraindikationen der rtPA sind nicht evidenzbasiert; aktuelle Daten aus Beobachtungsstudien zeigen beispielsweise, dass auch bei Patienten unter einer Therapie mit direkten oralen Antikoagulanzien eine systemische Thrombolysetherapie möglich ist. Tenecteplase (TNK) ist ein alternatives Thrombolytikum mit einigen pharmakologischen Vorteilen. Neueste Leitlinien weisen darauf hin, dass TNK insbesondere bei Patienten, die in Kombination mit einer endovaskulären Schlaganfalltherapie (EST) behandelt werden, gegenüber rtPA von Vorteil ist. Die Kombination aus IVT und EST soll vor allem im Zeitfenster von 4,5 h bei Patienten ohne Kontraindikationen erfolgen, im späteren Zeitfenster ist eine alleinige EST denkbar, wenn diese ohne Zeitverzug möglich ist.
