Analyse und Möglichkeiten der Optimierung von Prämedikationsgesprächen an einem Universitätsklinikum

Hintergrund

Das Prämedikationsgespräch als Risikoevaluation vor einem operativen Eingriff ist ein wichtiges Element der perioperativen Betreuung. Bisher liegen wenige Arbeiten vor, die sich mit der Analyse der Abläufe und Probleme in Prämedikationsambulanzen befassen.

Ziel der Arbeit

Es sollte prospektiv durch Erfassung sämtlicher Prämedikationsgespräche vor elektiven Eingriffen in der Prämedikationsambulanz in einem Zeitraum von mehreren Wochen ein großer Datenpool generiert und dieser im Hinblick auf Möglichkeiten einer Prozessoptimierung analysiert werden.

Material und Methoden

An 38 Werktagen wurde in der Prämedikationsambulanz des Universitätsklinikums Regensburg eine Erfassung der Wartezeiten und der Dauer des Prämedikationsgesprächs durchgeführt. Zudem wurden Patientenalter, ASA-Score (Klassifikation der American Society of Anesthesiologists), operierende Fachabteilung, Risikoklassifikation des operativen Eingriffs, Berufserfahrung des prämedizierenden Arztes, geplanter Zeitpunkt des operativen Eingriffs und Freigabestatus dokumentiert. Bei dem Prämedikationsgespräch auftretende Probleme wurden mithilfe eines Fragebogens registriert.

Ergebnisse

Insgesamt konnten 2233 Prämedikationsgespräche analysiert werden. Die Zahl der in der Prämedikationsambulanz vorgestellten Patienten differierte im Tagesverlauf stark und nahm am Ende der Woche insgesamt ab. Von Anästhesisten mit einer Berufserfahrung von mehr als 5 Jahren bzw. Fachärzten wurden Patienten signifikant häufiger freigegeben als von jüngeren Kollegen. Häufigster Grund für eine Freigabe mit Einschränkung oder keine Freigabe war das Fehlen von Laborbefunden oder Elektrokardiogramm (EKG). Als häufigster Grund für Probleme beim Prämedikationsgespräch wurde das Fehlen von Unterlagen genannt, die gemäß hausinternem Standard hätten vorliegen sollen. Die mittlere Wartezeit für die Patienten betrug 58,6 ± 30,3 min; Durchsicht der mitgebrachten Unterlagen und das Gespräch mit dem Patienten dauerten zusammen im Mittel 33,6 ± 16,3 min. Anästhesisten mit einer Berufserfahrung von 2 bis 5 Jahren benötigten für die Durchsicht der Unterlagen und das Prämedikationsgespräch signifikant kürzer als Berufsanfänger und erfahrene Kollegen. Zudem korrelierte die Dauer mit dem ASA-Score und dem Risiko des operativen Eingriffs.

Schlussfolgerung

Optionen zur Prozessoptimierung in der Prämedikationsambulanz stellen die Einführung von Reglementierungen zur Schaffung eines homogenen Patientenaufkommens im Tagesverlauf und das Einwirken auf die zuweisenden Kliniken, dass Patienten im Regelfall nur mit vollständigen Unterlagen gemäß Prämedikationsstandard vorgestellt werden, dar.
