Antikoagulation bei Vorhofflimmern

Zusammenfassung

Die neuen Leitlinien empfehlen basierend auf dem neuen CHA2DS2-VASC-Score eine therapeutische Antikoagulation bereits ab einem geringeren Risikoprofil als bisher. Durch das Hinzukommen neuer Antikoagulanzien erweitern sich daneben die Therapieoptionen. Zurzeit sind Vitamin-K-Antagonisten noch die Standardpräparate. Allerdings ist diese Therapie bei einigen Patienten problembehaftet. Das erhöhte Blutungsrisiko und die engmaschig erforderlichen Gerinnungskontrollen führen zudem dazu, dass viele eigentlich antikoagulationspflichtige Patienten nicht behandelt werden. Die neuen Antikoagulanzien Apixaban, Rivaroxaban und Dabigatran wurden mit dem Ziel entwickelt, diese Nachteile zu umgehen. Dabigatran hat die Zulassung für die Thromboembolieprophylaxe bei Patienten mit Vorhofflimmern bereits erhalten. Allen 3 Substanzen ist gemein, dass auf regelmäßige Blutentnahmen zur Wirksamkeitskontrolle verzichtet werden kann. Ob diese Einsparungen im Zusammenspiel mit den Kosteneinsparungen durch Verhinderung von ischämischen und Blutungsereignissen den deutlich höheren Preis im Vergleich zu Vitamin-K-Antagonisten rechtfertigen werden, müssen sozioökonomische Untersuchungen zeigen.
