Das Geschlecht in der Infektiologie – wie Geschlechtsunterschiede die Immunantwort auf Infektionen beeinflussen

Zusammenfassung

Die humorale und zelluläre Immunantwort auf Antigenstimulation, Impfungen und Infektionen unterscheidet sich zwischen Frauen und Männern. Genetische, epigenetische und hormonelle Determinanten spielen eine Rolle in der geschlechtsspezifischen Immunität. Die Expression von X‑chromosomalen Genen und die Wirkung von Sexualhormonen beeinflussen die Immunabwehr gegen eine Infektion maßgeblich. Dabei weisen Frauen stärkere humorale und zelluläre Immunantworten gegen Infektionen auf als Männer. Jedoch führt diese verstärkte Reaktivität gehäuft zu aberranten Entzündungsreaktionen und Autoimmunerkrankungen. Grundsätzlich sind Männer anfälliger für Infektionen mit Bakterien, Viren und Pilzen und zeigen gehäuft schwere Krankheitsverläufe. Frauen hingegen entwickeln häufiger unerwünschte Reaktionen auf Impfungen. Um die multiplen geschlechtsspezifischen Faktoren mit Einfluss auf das Immunsystem besser identifizieren zu können, sollten geschlechtsspezifische Unterschiede möglichst differenziert untersucht werden. Das bessere Verständnis geschlechtsspezifischer Unterschiede der Immunantwort wird langfristig Einfluss auf Prävention, Diagnostik und Therapie von Infektionskrankheiten haben und letztlich zur besseren Gesundheitsversorgung von Frauen und Männern beitragen.
