„Rapid-deployment“- und kathetergestützte Aortenklappenprothesen

Vergleichende Untersuchung der hämodynamischen Eigenschaften

Zusammenfassung

Die Therapie der hochgradigen, symptomatischen Aortenklappenstenose durchläuft aktuell einen rasanten Wandel. Aufgrund der exponentiell wachsenden Expertise auf dem Feld der kathetergestützten Verfahren wird der Goldstandard, sofern es diesen noch gibt, zunehmend infrage gestellt. Inspiriert durch diese Entwicklungen werden konventionelle Klappenprothesen dahingehend weiterentwickelt, dass diese mit verkürzten Operationszeiten implantiert werden können, um das Outcome des Patienten zu verbessern. Der vorliegende Beitrag stellt den retrospektiven Vergleich einer kathetergestützten Aortenklappenprothese mit einer chirurgischen „Rapid-deployment“-Prothese im Hinblick auf postoperative hämodynamische Eigenschaften dar. Transvalvuläre Druckgradienten und die paravalvuläre Regurgitation wurden als wichtige Surrogatparameter für das Langzeitüberleben der Patienten untersucht. Darüber hinaus wurden beide Klappentypen in einem In-vitro-Modell mithilfe der 4D-Fluss-MRT hinsichtlich ihres Strömungsprofils untersucht.
