5. Aortokoronare Bypassoperation (ACB-Operation)

Zusammenfassung

Die aortokoronare Koronarbypassoperation (ACB-Operation) ist die häufigste Herzoperation. Von ihr profitieren v. a. Hochrisikopatienten, CCS-III- und IV-Patienten sowie Patienten mit Dreigefäßerkrankung. Bei der Operation wird der stenotische Gefäßbereich mit einem neu implantierten Gefäß (Graft) überbrückt. Autologe arterielle Grafts werden gegenüber autologen V.-saphena-Grafts bevorzugt, weil ihre Verschlussrate viel geringer ist. Standard ist die Operation am nicht schlagenden Herzen unter Einsatz der Herz-Lungen-Maschine. Bei der Anästhesie muss v. a. das myokardiale Sauerstoffgleichgewicht durch strikte Kontrolle hämodynamischer Parameter (Herzfrequenz, Blutdruck, Kontraktilität) aufrechterhalten werden. Opioidbasierte Anästhetietechniken, supplementiert mit Propofol oder volatilen Anästhetika in niedriger Konzentration, sind Standard. Angestrebt wird eine frühe Extubation auf der Intensivstation (Fast-track). Die mittlere Behandlungsdauer im Krankenhaus beträgt ca. 2 Wochen.
