Karotisstenose - Wie abklären? Wann operieren?

Zusammenfassung

Die Häufigkeit höhergradiger Stenosen (> 50%) der Arteria (A.) carotis interna (ACI) liegt im mittleren Lebensalter bei etwa 1-3% und steigt im höheren Lebensalter auf 6-15% an. Während asymptomatische Stenosen ein relativ geringes Schlaganfallrisiko aufweisen, sind symptomatische Stenosen als Hochrisiko-Konstellation zu betrachten. Hier liegt das Schlaganfallrisiko bei 15-25% in den ersten Wochen nach dem ersten ischämischen Ereignis. Basis jeder Schlaganfallprävention bei einer Karotisstenose ist ein optimales Management der bestehenden vaskulären Risikofaktoren. Die Indikation zu einer Revaskularisierung bei höhergradigen asymptomatischen Stenosen ist individuell kritisch zu stellen; die Schlaganfall-/Letalitätsrate sollte unter 2% liegen. Die Revaskularisierung einer symptomatischen Stenose stellt eine effektive Schlaganfall-Sekundärprävention dar. Sie sollte innerhalb der ersten 14 Tage nach dem Erstereignis durchgeführt werden und eine Schlaganfall-/Letalitätsrate unter 4% haben. Wegen ihrer geringeren periprozeduralen Komplikationsrate im Vergleich zur Karotisangioplastie mit Stenting ist die Kartotisendarterektomie die Methode der ersten Wahl.
