Ein White Paper anlässlich der ersten europäischen Leitlinien 2023

Zusammenfassung

Die Moyamoya-Angiopathie ist eine sehr seltene intrakraniale nicht-entzündliche Vasopathie, die durch Stenosen und Verschlüsse der intrakranialen Abschnitte der Aa. carotides internae, cerebri mediae und anteriores gekennzeichnet ist. Namensgebend ist das kompensatorische Kollateralnetz. Während die Erkrankung in Japan, Korea und China bekannter ist, ist in Europa eine hohe Fehldiagnoserate beschrieben, und es wird von einer höheren Dunkelziffer ausgegangen. Klinische Manifestationen sind hämodynamische sowie embolische zerebrale Ischämien, oft auch wiederholte getriggerte transitorisch ischämische Attacken, zerebrale Blutungen, Kopfschmerzen, Bewegungsstörungen und kognitive Defizite. Eine frühe Detektion sowie eine fundierte Indikationsstellung einer zielführenden Therapie, ob konservativ oder mittels Bypass-Operation, erscheint bei dem hohen Risiko von Behinderung bis hin zu tödlichen Komplikationen wichtig.
