Schlaganfallakuttherapie im Alter

Zusammenfassung

Bei Patienten im Alter über 80 Jahren sind 4 der 5 evidenzbasierten Akuttherapien des ischämischen Schlaganfalls (Stroke-Unit-Behandlung, Plättchenhemmergabe, intravenöse Thrombolyse [IVT] und mechanische Thrombektomie [MT]) wirksam, allerdings mit höherer Morbidität als bei jüngeren Patienten. Die für diese Maßnahmen gegebene Indikation muss sich in Fällen der IVT und der MT an der jeweiligen Komorbidität des Patienten orientieren. Nach einem Scheitern dieser Verfahren ist der konsequente Therapiezielwechsel zum palliativen Vorgehen angemessen. Die dekompressive Kraniotomie bei raumfordernden Mediainfarkten kann bis zur relativen bzw. absoluten Altersgrenze des 60. bzw. 70. Lebensjahrs indiziert sein. Patienten im Alter über 80 Jahren erhalten oft keine IVT und MT. Zwar weist die deutsche Zulassung für Alteplase im Rahmen der IVT bei einem Alter über 80 Jahren auf die sorgfältig vorzunehmende Indikationsprüfung hin, die Anwendbarkeit ist jedoch als grundsätzlich gegeben zu erachten.
