„No-option critical limb ischemia“: Welche Alternativen zur Amputation gibt es?

Zusammenfassung

Hintergrund

Ein Voranschreiten der Atherosklerose führt bei vielen Patienten dazu, dass immer weniger Möglichkeiten für eine arterielle Revaskularisation verbleiben, sodass letztlich die Amputation droht.

Fragestellung

In diesem Übersichtsartikel werden die Optionen für die Patientenversorgung vorgestellt und die klinische Entscheidungsfindung erörtert, wenn die arterielle Anatomie einen konventionellen chirurgischen oder interventionellen Eingriff nicht mehr zulässt.

Material und Methode

Anhand einer extensiven Literaturrecherche wurden wissenschaftliche Berichte zu alternativen Verfahren zum Thema kritische Extremitätenischämie ohne Revaskularisationsmöglichkeiten („no-option critical limb ischemia“; „no-option CLI“) zusammengetragen und deren Ergebnisse zur Vermeidung einer Amputation evaluiert. Nicht berücksichtigt wurden medikamentöse Strategien.

Ergebnisse

Insgesamt konnten acht verschiedene Strategien zur Amputationsvermeidung identifiziert und bewertet werden (Zelltherapie, Gentherapie, Sympathikolyse/Sympathektomie, Rückenmarksstimulation [SCS], hyperbare Oxygenierung [HBOT], hypertensive extrakorporale Extremitätenperfusion (HELP), biomechanische sequenzielle Kompressionstherapie [SCBD] und tiefe Venenarterialisation [DVA]).

Schlussfolgerungen

Trotz des überwiegend experimentellen Charakters der hier vorgestellten Methoden stehen immer mehr Instrumente zur Verfügung, um einen No-option-CLI-Patienten extremitätenerhaltend zu behandeln. Dieser Übersichtsartikel gibt einen aktuellen Überblick über Möglichkeiten der Amputationsvermeidung durch verschiedene Techniken, mit deren Risiken und Ergebnissen.

S3-Leitlinie „Diagnostik und Therapie des Karpaltunnelsyndroms“

Prof. Dr. med. Gregor Antoniadis Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
