Virusassoziierte Arthritiden

Zusammenfassung

Epidemiologische Studien legen nahe, dass etwa 1 % der akuten Arthritiden eine virale Genese aufweisen. Die arthritogene Wirkung der Virusinfektion kann dabei aus der viralen Infektion synovialer Zellen, aus der humoralen oder zellulären Immunantwort auf virale Antigene oder aus Autoimmunprozessen resultieren. Die Gelenkmanifestationen können in ihrem Befallsmuster eine rheumatoide Arthritis imitieren und werden häufig von einem Exanthem und einer grippalen Symptomatik begleitet. Die Diagnose einer viralen Arthritis stützt sich in erster Linie auf serologische Verfahren wie den pathogenspezifischen Nachweis von IgM- und IgG-Antikörpern. Die Virusisolation oder der Nukleinsäurenachweis mittels Polymerasekettenreaktion aus Synovialgewebe und -flüssigkeit ist aufwendig, gelingt nur selten und ist nicht immer beweisend für eine virale Genese der Arthritis. Meist, aber nicht in allen Fällen, verlaufen die viralen Arthritiden selbstlimitierend.
