Umstellungsosteotomien bei Arthrose am Sprunggelenk (supra- und inframalleolar) – Was kann man erwarten?

Zusammenfassung

Das Ziel einer supramalleolaren Umstellungsosteotomie (SMOT) ist, die Beschwerden des Patienten zu reduzieren und das Fortschreiten der Arthrose zu verhindern bzw. zu verlangsamen. Die Indikation für eine SMOT ist die asymmetrische Arthrose des oberen Sprunggelenks (OSG) mit Varus- oder Valgusdeformität. Gute klinische Ergebnisse können bei einer Arthrose im frühen bis mittleren Stadium erzielt werden. Neben den zu- und aufklappenden Osteotomien hat die Dome-Osteotomie ihre Indikation v. a. bei größeren Deformitäten. Die präoperative Anfertigung von patientenindividuellen Schnittblöcken erleichtert die intraoperative Umsetzung und scheint die Präzision zu erhöhen. Hierzu sind weitere Studien notwendig. Die Notwendigkeit für eine additive Fibulaosteotomie wird weiterhin diskutiert. Zusätzliche inframalleolare Osteotomien und Weichteileingriffe sind zur Balancierung des Fußes häufig notwendig.
