Hausärztliche Versorgungsqualität und Lebensqualität aus Sicht älterer Patienten mit Gon- und Coxarthrose

Ergebnisse aus der Kohortenstudie MobilE-TRA

ZUSAMMENFASSUNG

Hintergrund: In der Versorgung von Arthrosepatienten spielt die allgemeinmedizinische Praxis eine zentrale Rolle. Ziel der Forschungsarbeit ist, den Zusammenhang der hausärztlichen Versorgungsqualität aus Perspektive der Patientinnen und Patienten mit deren gesundheitsbezogener Lebensqualität, arthrosespezifischen Symptomen und körperlicher Funktionsfähigkeit zu untersuchen.
Methode: Rekrutiert wurden 111 Cox- und/oder Gonarthrosepatientinnen und -patienten ab 65 Jahren aus Hausarztpraxen in Sachsen und Bayern. Die Studienteilnehmer erhielten einen papierbasierten Fragebogen. Dieser untersuchte die soziodemografischen Daten, Depressivität (PHQ-9), körperliche Aktivität und gesundheitsbezogene Lebensqualität (EQ-5D mit EQ-VAS) sowie die von den Patientinnen und Patienten subjektiv bewertete hausärztliche Versorgungsqualität (PACIC short form). Zudem wurden die arthrosespezifischen Symptome (Schmerz, Gelenksteifigkeit) und die körperliche Funktionsfähigkeit (WOMAC) ermittelt. Die Analyse erfolgte mittels multipler linearer Regressionsmodelle.
Ergebnisse: Ein größeres Ausmaß an Depressivität war mit stärkeren Schmerzen (beta = 0,378; 95%-KI [0,180; 0,576]), ausgeprägterer Gelenksteifigkeit (beta = 0,223; 95%-KI [0,135; 0,310]) und reduzierter körperlicher Funktionsfähigkeit (beta = 1,628; 95%-KI [0,908; 2,348]) assoziiert. Je älter (beta = -0,901; 95%-KI [-1,705; -0,097]) und je depressiver (beta = -1,654; 95%-KI [-2,820; -0,488]) der Arthrosepatient war, desto niedriger war dessen gesundheitsbezogene Lebensqualität. Es konnte kein signifikanter Zusammenhang zwischen der hausärztlichen Versorgungsqualität aus Perspektive der Patienten und deren Lebensqualität, Symptomen oder körperlicher Funktionsfähigkeit festgestellt werden.
Schlussfolgerungen: In der Literatur konnte mehrfach ein Zusammenhang zwischen der Qualität der Versorgung von chronisch Kranken und deren Lebensqualität sowie anderen gesundheitlichen Aspekten festgestellt werden. Die Ergebnisse dieser Studie konnten dies nicht bestätigen. Da die Depressivität mit einer niedrigeren Lebensqualität sowie ausgeprägteren Beschwerden der Arthrosepatientinnen und -patienten assoziiert war, sollte besonders bei dieser Patientengruppe auf eine mögliche depressive Begleiterkrankung geachtet und diese gegebenenfalls behandelt werden.
