Arthroskopische Möglichkeiten biorekonstruktiver Verfahren bei Knorpelschäden der Schulter

Zusammenfassung

Chondrale bzw. osteochondrale Läsionen des Schultergelenks können aufgrund fehlender Regeneration des Gelenkknorpels zu vorzeitiger Omarthrose führen. Steigendes Gesundheitsbewusstsein, hoher Aktivitätsanspruch und technischer Fortschritt steigern daher das Interesse an knorpelregenerierenden minimalinvasiven Techniken zu einem glenohumeralem Gelenkerhalt oder der Verzögerung einer prothetischen Versorgung. Gegenüber der überzeugenden Datenlage biorekonstruktiver Verfahren bei Knorpelschäden des Kniegelenks mit meist guten mittelfristigen Ergebnissen sind Studien zur innovativen Therapie glenohumeraler Knorpelläsionen bisher rar. Gegenwärtige Techniken wie knorpelstimulierende Verfahren zur mesenchymalen Stammzellrekrutierung (z. B. Mikrofrakturierung), die (autologe) Knochen-Knorpel-Transplantation, die (matrixgekoppelte) autologe Chondrozytentransplantation und der biologische Oberflächenersatz adressieren den glenohumeralen fokalen Knorpelschaden bis hin zum ausgedehnten osteochondralen Defekt. Durch zunehmende arthroskopische Einsetzbarkeit verstärken sie das Einsatzpotenzial des Schulterchirurgen bedeutsam. Künftige Studien zur evidenzbasierten Anwendbarkeit werden die Wahl des geeigneten Verfahrens standardisieren.
