ASNM intraoperative SSEP position statement

The updated ASNM position statement on intraoperative somatosensory evoked potentials (SSEPs) is a significant improvement on the previous version. However, misconceptions on stimulation repetition rate and interleaving in the previous version have recurred in this updated version. The perpetuation of these misconceptions is likely due to the absence of predicate literature when the previous version was composed, but since then, two recent articles shed light on these fundamental concepts which are foundational to acquisition of high quality, high quantity SSEPs.…Stimulation rates between 2 and 5 Hz are recommended. However, lower stimulation rates (between 1.5 and 3 Hz) can sometimes improve poor lower extremity responses, particularly when compromise of neurological function (e.g. neuropathy) is present, whereas upper extremity SEPs may demonstrate little or no change at stimulation rates as high as 9 Hz. Increasing the stimulus rate beyond 9 Hz for the upper extremity SEPs and 5 Hz for the lower extremity typically results in a substantial degradation of the SEPs, particularly in cortical responses.” (ASNM 2024 SSEP Position Statement).
