Pancreaticoduodenectomy (PD), which has been performed most frequently for ampullary carcinoma, still causes a nonnegligible rate of surgical complications.
1 A less invasive local resection, transduodenal ampullectomy (TDA), has been attempted for early stage cancers confined to the ampulla of Vater (AoV) or sphincter of Oddi (carcinoma in situ or pT1). Certain centers have performed TDA with lymph node dissection (LND) and achieved oncologic outcomes comparable to PD in early AoV cancer.
2,3 These studies suggest that TDA with LND might be a viable surgical treatment for patients unfit for PD. However, the extent of adequate regional LND is still unknown, and previous studies have been limited by sample size or short duration of follow-up. Accordingly, the role of TDA in early AoV cancers remains inconclusive. …
