Achieving negative surgical margins is widely recognized as the most significant predictor of improved long-term survival after pelvic exenteration.
1,2 Over the past two decades, multidisciplinary perioperative management alongside enhanced cross-sectional imaging, refined surgical techniques, and improved neoadjuvant therapies have contributed to an increase of R0 resection rates.
1 A recent international quality improvement initiative, PelvEx, set benchmarking standards for pelvic exenteration centers, specifying a minimum surgical volume of ≥ 20 partial of total pelvic exenterations per year and an R0 resection rate of ≥ 79% for primary rectal cancer.
