Repeating advanced regional therapies such as cytoreductive surgery (CRS) with hyperthermic intraperitoneal chemotherapy (HIPEC) after recurrence of appendiceal mucinous peritoneal carcinomatosis is a common practice with an acceptable safety profile.1,2 However, most of the data behind this practice are based on limited studies with mixed populations, so the decision to repeat CRS/HIPEC is likely based on resectability and patient status.3 Currently, there is no consensus on the best salvage treatment. Some patients are managed with systemic therapy, but the lack of an acceptable response to this alternative has led some peritoneal malignancy groups to repeat CRS/HIPEC regardless of tumor biology.1‐4 Thus, evidence behind this practice is still evolving, and much about patient selection, the effect of tumor biology, safety, feasibility, and the fate of non-candidate patients remains unknown. …
