Extramural vascular invasion (EMVI) and tumor deposits (TD) are poor prognostic factors in rectal cancer (RC), especially when resistant to neoadjuvant therapy. Neoadjuvant chemotherapy (NAC) non-responders with residual EMVI and TD after NAC have higher local and distant recurrence rates even after R0 resection, whereas NAC-effective RCs in which EMVI and TD disappeared after NAC have a good prognosis.
