Liver resection is the standard treatment for intrahepatic cholangiocarcinoma (ICC), but, in patients with aggressive tumor biology or high tumor burden, the surgical indication should be weighed against the risk of early recurrence (up to 50% at the 6-month follow-up) and the increasing effectiveness of new systemic therapies.
1 Therefore, an accurate preoperative prognostic assessment is mandatory. Standard morphology-based scores have limited reliability and genetic data do not yet guide treatment planning.
