Up to one third of paraganglioma cases are linked to germline mutations; the autosomal-dominant hereditary paraganglioma-pheochromocytoma syndrome (HPPS) is most common.1 This condition often involves mutations in the SDHx gene complex. Specifically, a deleterious mutation in SDHB demonstrates high penetrance, highlighting the importance for surgical oncologists to understand its strong association with metastatic disease and unfavorable prognosis.1 Historically, concern for aggressive behavior and the difficultiess in achieving complete resection favored an open approach over minimally invasive surgery (MIS) for SDHB-mutated paragangliomas.2 This recommendation, however, might require reevaluation, because it was made during the laparoscopic era before all advancements in robotic technique were explored and studied. The robotic approach offers superior visualization, and increased degrees of freedom in manipulation and precision is beneficial for navigating the complex retroperitoneal anatomy where paragangliomas commonly occur.1,3 Given the meticulous tumor dissection and vital structure preservation, robotic surgery presents a compelling argument for use in managing even the high-risk variants of paragangliomas, with the potential for lower morbidity and quicker patient recovery. …
