Main pancreatic duct (MPD) width in preoperative computed tomography (CT) imaging has been widely accepted to predict postoperative pancreatic fistula (POPF).
1 It is easy to measure and intuitively understandable to have an inverse correlation with the stiffness of pancreatic parenchyma. In other words, MPD dilatated cases have a hard pancreas and are likely to avoid grade B/C POPF. However, we cannot apply MPD measurement to middle or distal pancreatectomy cases because tumor-induced MPD dilatation does not affect the remnant pancreas texture. In addition, we sometimes experience relatively solid pancreatic parenchyma cases without MPD dilatation who underwent a good postoperative course without POPF. …
