Asphyxierende Thoraxdysplasie (Jeune-Syndrom)

Zusammenfassung

Die asphyxierende Thoraxdysplasie (Jeune-Syndrom) wurde 1954 erstmals von Jeune et al. beschrieben. Es handelt sich um ein autosomal-rezessives Leiden unterschiedlicher Expression mit Fehlanlage des Chromosoms 15q13. Die Symptome bestehen in einem schmalen langen Thorax, kurzen Rippen, einer ausgeprägten Hypoplasie der Lunge mit schwerer Asphyxie, weiterhin Kleinwuchs, kurzen Extremitäten sowie in vielen Fällen einer Niereninsuffizienz durch Nephronophthise, fibrozystischen Veränderungen an Leber und Pankreas sowie einer Hyperbilirubinämie. Die Mortalität ist bei Ausprägung aller Symptome sehr hoch. Als operative Verfahren sind die mediane Sternotomie mit Interposition einer Methylmethacrylatprothese nach Todd sowie neuerdings die laterale Thoraxexpansion nach Davis beschrieben. Langzeitbeobachtungen sind selten. Aus diesem Grund wird hier der Fall eines jetzt 5-1/2-jährigen Mädchens mit weitgehender Normalisierung der Beatmungsparameter nach 2-maliger Thoraxerweiterungsplastik vorgestellt.
