Asthma bronchiale bei Kindern und Jugendlichen

Zusammenfassung

Asthma bronchiale ist die häufigste chronische Lungenerkrankung im Kindes- und Jugendalter, jedoch immer noch unterdiagnostiziert. Im Vergleich zu Erwachsenen besteht bei Kindern häufiger eine atopische Disposition. Die Diagnose basiert in erster Linie auf klinischen Kriterien. Die Behandlung folgt einem Stufentherapieplan, in dem inhalative Kortikosteroide die Basis der antientzündlichen Therapie bilden. Im Vergleich zu Erwachsenen wird bereits bei niedrigeren Dosen des inhalativen Kortikosteroids eine Kombination mit einer weiteren Substanzklasse empfohlen. Montelukast hat in der Basistherapie einen höheren Stellenwert. Vor dem Schulalter ist eine definitive Diagnose schwierig; daher wird das Risiko für ein späteres Asthma abgeschätzt und die Therapie nach dem aktuellen Beschwerdebild gesteuert. Ziel der Behandlung in jedem Lebensalter ist es, eine gute Asthmakontrolle zu erreichen und zu erhalten und eine krankheitsbezogen unbeeinträchtigte körperliche und psychosoziale Entwicklung der betroffenen Kinder und Jugendlichen zu sichern.
