Astigmatische Keratotomie mit dem Femtosekundenlaser

Korrektur hoher Astigmatismen nach Keratoplastik

Zusammenfassung

Hintergrund

Ziel der Arbeit war es, retrospektiv die Ergebnisse einer Fallserie femtosekundenlaserassistierter astigmatischer Keratotomien (Femto-AK) nach perforierender Keratoplastik zu analysieren.

Patienten und Methoden

Es wurde an 10 Augen mit hohem Astigmatismus nach Keratoplastik eine paarige bogenförmige astigmatische Keratotomie mit einem 60-kHz-Femtosekundenlaser (IntraLaseTM, Abbott Medical Optics, Irvine, Kalifornien, USA) durchgeführt. Zielgrößen waren der prä- und postoperative unkorrigierte und bestkorrigierte Visus, die subjektive Refraktion, topographische Daten und die Werte der aus der Hornhautvorderfläche berechneten kornealen Wellenfront.

Ergebnisse

Alle Femto-AK konnten ohne Komplikationen durchgeführt werden. Die präoperative mittlere Sehschärfe (logMAR) betrug s. c. 1,27 und c. c. 0,55, der mittlere subjektive Zylinder −7,4 dpt und der mittlere topometrische Astigmatismus 9,3 dpt. Die postoperative mittlere Sehschärfe (logMAR) betrug s. c. 1,12 und c. c. 0,47, der mittlere subjektive Zylinder −4,1 dpt und der mittlere topometrische Astigmatismus 6,5 dpt. Die Veränderungen der kornealen Aberrationen höherer Ordnung wiesen eine erhebliche Streubreite auf und waren daher nicht signifikant.

Schlussfolgerungen

Die Femto-AK stellt eine sichere und effektive Methode zur Reduktion höherer kornealer Astigmatismen dar. Aufgrund der Biomechanik der Hornhaut und fehlender Erfahrungswerte mit der vorgestellten Technologie sind höhere Patientenzahlen zur Entwicklung optimierter Nomogramme erforderlich.
