Postextubationsdysphagie bei Intensivpatienten

Aktuelle Erkenntnisse und klinische Empfehlungen

Zusammenfassung

Hintergrund

Eine relevante Anzahl an kritisch kranken, beatmeten Intensivpatienten zeigt nach der Extubation eine ausgeprägte Dysphagie. Einige Studien konnten nachweisen, dass eine Postextubationsdysphagie (PED) zur erheblichen Verschlechterung des Outcomes führt. Die Sensibilisierung hinsichtlich dieses Störungsbildes, die frühzeitige Diagnostik und die Implementation geeigneter Therapiemaßnahmen auf Intensivstationen sind daher von großer Bedeutung.

Fragestellung

Aktueller wissenschaftlicher und klinischer Kenntnisstand zur PED, diagnostische Möglichkeiten, therapeutische Maßnahmen sowie die Entwicklung von konkreten Empfehlungen für die klinische Praxis.

Methode

Es wurde eine selektive Literaturrecherche in PubMed, MEDLINE und Cochrane nach Schlagwörtern durchgeführt.

Ergebnisse

Die Inzidenz einer PED wird in der Literatur sehr heterogen angegeben, beträgt vermutlich jedoch mindestens 10 % bei Intensivpatienten nach einer Beatmung. Die Intubationsdauer spielt dabei eine entscheidende Rolle. Als Ursache wird ein Zusammenspiel mehrerer Faktoren angenommen, wobei der Beeinträchtigung laryngealer Strukturen besondere Bedeutung zukommt. Die PED führt u. a. zu einem längeren Krankenhausaufenthalt, einer höheren Mortalität, mehr Reintubationen sowie einer höheren Anzahl an sondenernährten Patienten. Als diagnostische Möglichkeiten werden pflegerisch durchgeführte Screenings, die klinische Schluckuntersuchung sowie insbesondere die apparative Untersuchung via endoskopischer Schluckuntersuchung (FEES) empfohlen. Die Therapie sollte adaptive Maßnahmen im Sinne einer angepassten Kostform, aber auch funktionelle Übungen beinhalten. Innovative Ansätze wie neuronale Stimulationstechniken sind ebenfalls denkbar. Ziel ist vorwiegend die Vermeidung von Penetration und Aspiration, um respiratorischen Komplikationen entgegenzuwirken.

Schlussfolgerung

Auf einigen Intensivstationen ist das Störungsbild einer PED trotz eindeutiger Studienergebnisse noch wenig präsent. Ein einfacher Algorithmus in der Behandlung von Intensivpatienten kann zur frühzeitigen Erkennung beitragen und weitere Handlungsschritte einleiten. Konkrete diagnostische und therapeutische Empfehlungen sollten Eingang in die klinischen Behandlungsstandards finden.
