Volumetrische Kapnographie zur Analyse und Optimierung von Ventilation und Gasaustausch

Zusammenfassung

Die Kapnographie als grafische Darstellung der exspiratorischen Kohlenstoffdioxid(CO2)-Konzentration, ist, neben der Pulsoxymetrie, essenzieller Bestandteil des Monitorings eines jeden beatmeten Patienten. Die Kapnographie stellt die CO2-Kinetik auf nichtinvasive Art und in Echtzeit dar. Sie dient in der täglichen Anästhesieroutine hauptsächlich zur Identifikation der korrekten Intubation und zur Anpassung des zu applizierenden Atemminutenvolumens. Die Kapnographie kann aber, insbesondere in ihrer bisher klinisch noch nicht so weit verbreiteten Form der volumetrischen Kapnographie (VCap) viel weitreichendere und klinisch besonders wertvolle zusätzliche Informationen liefern. Hierzu zählen Überwachung und Optimierung der Ventilation sowie Beurteilung des Gasaustausches. Im vorliegenden Beitrag werden Parameter zur Entscheidungsfindung am Krankenbett vorgestellt, die bislang nur durch aufwendigere, invasivere, nichtautomatisierte Verfahren gewonnen werden konnten.
