3. Atopisches Ekzem und klinische Sonderformen

Zusammenfassung

Das atopische Ekzem ist die häufigste chronische Hauterkrankung des Kindesalter.Etwa 85 % aller Erkrankungen manifestieren sich bereits bis zum 5. Lebensjahr. Die Diagnose wird klinisch gestellt, ein erkrankungsspezifischer Laborparameter fehlt. Die Hautbarriere ist in läsionaler, aber auch in klinisch unauffälliger Haut gestört. Die im Rahmen der Behandlung wichtige Basistherapie versucht eine Stabilisierung dieser Barrierefunktionsstörung. Für die antiekzematöse Therapie sind topische Steroide der 4. Generation und Calcineurininhibitoren die Therapeutika der Wahl. Orale sedierende Antihistaminika, Antibiotika oder auch Immunsuppressiva sind insbesondere bei schwer verlaufenden Fällen zeitlich begrenzt notwendig. Das Therapiemanagement beinhaltet auch die Abklärung der individuellen Provokationsfaktoren, wobei irritative Auslöser zahlenmäßig die allergischen deutlich übertreffen. Die Teilnahme an Schulungsprogrammen ist für ein verbessertes Krankheitsverständnis und eine verbesserte Krankheitsakzeptanz empfehlenswert.
