Autoimmune Hemolytic Anemia in a Pediatric Patient with Chronic Vivax Malaria

To the Editor: Malaria is an important infective cause of anemia in tropical countries. We present an eight-year-old boy with complaints of gradually progressive abdominal distension, with occasional fever, and weakness since 2 y without any history of jaundice or bleeding tendencies. He was transfused two units of packed cells five months back. Examination revealed tachycardia, tachypnea, severe pallor, growth retardation, hepatomegaly, and massive splenomegaly. His Hb was 5.2 g/dl, MCV 94.1 fl, with normal MCH, MCHC, WBC and platelets, ESR 116 mm/h, and reticulocyte count 2.2%. Renal & liver function tests, Widal, urine routine examination, culture, and rK39 were normal. Peripheral smear revealed marked anisopoikilocytosis and spherocytes along with Plasmodium vivax parasite in all its stages. …
