Bedeutung der Muskulatur für die Entwicklung neuromuskulärer Deformitäten

Neue diagnostische und therapeutische Aspekte

Zusammenfassung

Der Sammelbegriff „neuromuskuläre Erkrankungen“ steht für eine Vielzahl von Diagnosen, die aufgrund ihrer Auswirkungen auf den Bewegungsapparat auch für den Orthopäden wichtig sind. Die Veränderungen von Muskelstruktur und Muskelfunktion sind maßgeblich für die kontinuierliche Entwicklung von Deformitäten und die daraus resultierenden Funktionsstörungen. Die diagnostischen und therapeutischen Maßnahmen sollten sich an der Bedeutung der Muskulatur für das jeweils vorliegende Störungsbild orientieren. Die klassische orthopädische Maßnahme der Muskel-/Sehnenverlängerung wird zunehmend durch eine funktionellere Betrachtungsweise abgelöst. Dabei werden die Muskelkraft, die Muskelexkursion, die Muskelstruktur und das Muskelgleichgewicht als die wesentlichen Komponenten für neuromuskuläre Deformitäten beachtet. Bei verminderter Muskelkraft muss abgeklärt werden, ob ausreichend innervierbare Muskulatur zur Verfügung steht. Anderenfalls sind auch nach einer Korrektur auf Dauer Orthesen notwendig. Neuromuskuläre Kontrakturen beinhalten unterschiedliche strukturelle Korrelate, denen allen eine verminderte Muskel- und Gelenkexkursion gemeinsam ist. Alle Korrekturen müssen sich an der Struktur und an der Funktion der betroffenen Muskeln orientieren. Jede Verlängerung der Muskel-Sehnen-Einheit führt zur Krafteinbuße. Ein gestörtes Muskelgleichgewicht lässt sich operativ balancieren, wobei anschließend Orthesen notwendig sind.
