Bedeutung der Pharmakogenetik für die tägliche Praxis

Zusammenfassung

Die Pharmakogenetik ist ein Teilgebiet der klinischen Pharmakologie und beschäftigt sich mit erblich bedingten Ursachen individuell unterschiedlicher Reaktionen auf Arzneistoffe. Mittels klinischer Studien und molekulargenetischer Methoden werden Faktoren untersucht, die die Wirksamkeit oder Toxizität von Arzneistoffen beeinflussen. Ziel der Pharmakogenetik ist, dem behandelnden Arzt eine auf die genetischen Merkmale seines Patienten zugeschnittene Pharmatherapie zu ermöglichen. Derzeit ist dieses Konzept am besten ausgereift und in der klinischen Praxis anwendbar für arzneimittelmetabolisierende Enzyme, wie z. B. dem Cytochrom P450 (CYP) 2D6, CYP2C19 oder der Thiopurinmethyltransferase (TPMT). Genetisch bedingte Aktivitätsminderungen dieser Enzyme führen bei Standarddosierungen zu häufigeren Nebenwirkungen bis zu toxischen Reaktionen mit dem Risiko eines tödlichen Ausgangs. Auch die Effektivität der Arzneimittelwirkung am Wirkort (z. B. Rezeptor) unterliegt genetischen Einflüssen. Prospektive kontrollierte klinische Studien werden benötigt, um den Wert der Pharmakogenetik für die Klinik und den Therapieausgang zu untersuchen und die Felder der klinischen Anwendbarkeit abzustecken.
