Zusammenfassung

Patienten, Wissenschaftler und Leistungserbringer sind zunehmend daran interessiert, Interventionen zu identifizieren, die nicht nur die Mortalität reduzieren, sondern auch Symptome, Funktion und gesundheitsbezogene Lebensqualität verbessern. Die gesundheitsbezogene Lebensqualität ist ein starker, unabhängiger Prädiktor für Mortalität, kardiovaskuläre Ereignisse, Krankenhausaufenthalte und Behandlungskosten bei Patienten mit kardialen Erkrankungen. Eine Fernüberwachung von Schrittmachern hat einen positiven Effekt auf die gesundheitsbezogene Lebensqualität und funktionelle Kapazität und ist der Überwachung dieser Patienten in Krankenhäusern gleichwertig. Eine ICD(implantierbarer Kardioverter-Defibrillator)-Implantation hat Auswirkungen auf das psychische Befinden der Patienten, wobei ein Großteil der Patienten die Angst vor ICD-Schockabgabe als für die Lebensqualität besonders beeinträchtigend empfindet. Variablen wie Alter, Geschlecht und Implantationsdauer sollten in der Beurteilung und der Planung von Strategien zur Verbesserung der Lebensqualität von Patienten mit kardialen elektronischen Implantaten beachtet werden.

SGLT-2-Inhibitoren für alle Patient:innen mit chronischer Herzinsuffizienz empfohlen

Das ESC-Leitlinien-Update 2023 bedeutet einen Paradigmenwechsel in der Behandlung der Herzinsuffizienz (HF), denn nun werden SGLT-2i sowohl für HFrEF, als auch für HFmrEF und HFpEF empfohlen. Somit können jetzt alle Patient:innen mit HF von SGLT-2i als zentralem Bestandteil der Therapie profitieren.

Dapagliflozin als neue Therapieoption zur Behandlung einer Herzinsuffizienz unabhängig von der Ejektionsfraktion

Dapagliflozin ist nun zur Behandlung aller Patient:innen mit chronischer symptomatischer Herzinsuffizienz zugelassen und bietet somit auch neue Hoffnung für die Therapie von jenen mit HFpEF. In der DELIVER-Studie zeigte der SGLT-2-Inhibitor eine signifikante Reduktion von Herzinsuffizienz-Hospitalisierungen und CV-Todesfällen.
