Zusammenfassung

Diese Arbeit befasst sich aus der Perspektive und anhand von Beispielen aus dem Bereich der angeborenen Stoffwechselkrankheiten mit der Frage, welchen Beitrag Leitlinien zur Standardisierung in der Medizin leisten können. Das Ziel von Leitlinien ist es, evidenzbasierte Empfehlungen auszusprechen, die informierte Entscheidungen ermöglichen und die Variabilität klinischen Handelns zu reduzieren, also eine Standardisierung herbeizuführen. Leitlinien, die mithilfe transparenter, strukturierter Prozesse der Konsensfindung erstellt sowie zielgruppennah und pragmatisch implementiert werden, haben höhere Chancen, von ihren Adressaten umgesetzt zu werden und damit das Ziel der Standardisierung zu erreichen. Leitlinien und Standardisierung sollten jedoch Kreativität und Autonomie von Arzt und Patient nicht ersticken. Abweichungen von der Leitlinie, um Anpassungen an die Individualität von Patienten und Situationen vornehmen zu können, sollten in die Diskussion zur ständigen Verbesserung der Leitlinie einfließen. Methodisch anspruchsvolle Leitlinienprozesse können aufdecken, zu welchen wichtigen Themen Evidenz fehlt und dadurch zu Forschung inspirieren, die diese Evidenzlücken schließen kann.
