Neue operative Techniken zur Therapie des benignen Prostatasyndroms

Zusammenfassung

In den letzten Jahren wurden neue operative Techniken zur Behandlung des benignen Prostatasyndroms (BPS) in die urologische Praxis eingeführt und in klinischen Studien evaluiert, mit denen Komplikationen der Standardverfahren (z. B. transurethrale Resektion der Prostata, TURP) vermieden und dennoch gleichwertige klinische Ergebnisse erzielt werden sollen. Im Vordergrund steht der Erhalt der erektilen Funktion und der antegraden Ejakulation. Außerdem sind ambulante Verfahren wünschenswert, die eine rasche Rekonvaleszenz ermöglichen. Diese Arbeit stellt Interventionsablauf, Wirkweise und aktuelle klinische Evidenz für neue mechanische Ansätze einer Desobstruktion, wassergestützte Ablationsverfahren und die Prostataarterienembolisation vor. Studienergebnisse deuten teilweise an, dass künftig nahezu jedem Patienten eine individualisierte Operationstechnik angeboten werden kann, die bei minimiertem Komplikationsrisiko eine optimale symptomatische und funktionelle Verbesserung mit sich bringt.
