26. Besonderheiten bei Kindern mit rheumatischen Erkrankungen

Zusammenfassung

Fragen zur Ernährung von Kindern mit autoimmunologischen Erkrankungen sind häufig und die Publikationen in der Laienpresse irreführend. Erschwerend wirken sich die medizinhistorisch begründete unscharfe Abgrenzung der rheumatoiden Arthritis von der Arthrose und der Arthritis urica (Gicht) aus. Validierte wissenschaftliche Untersuchungen, insbesondere bei Kindern und Jugendlichen, bilden die Ausnahme und vermitteln ein oft widersprüchliches Bild. Bei den meisten autoimmunologischen Erkrankungen fehlen kontrollierte diätetische Studien völlig. Bei den verschiedenen Formen kindlicher Arthritis, der juvenilen idiopathischen Arthritis (JIA, Einteilung nach Klassifikation der ILAR; Petty et al. 1998) liegen zwar Untersuchungen vor; hierbei wurden die einzelnen Untergruppen der Erkrankung jedoch häufig nicht differenziert betrachtet. Dennoch zeigte sich bei Kindern und Jugendlichen mit einer JIA oder einem juvenilen systemischen Lupus erythematosus (jSLE) eine signifikant erhöhte Zufuhr von Fetten und Eiweiß bei verminderter Zufuhr von Eisen, Zink und Vitamin A als vorteilhaft (Caetano et al. 2009). Grundsätzlich müssen bei rheumatischen Erkrankungen 3 diätetische Ansätze betrachtet werden: Eliminationsdiäten und Fastenkuren in der Absicht einer Reduktion entzündungsfördernder Nahrungsstoffe, Substitutionsdiäten mit dem Ziel der Zufuhr entzündungshemmender Nahrungsstoffe, Vitamine oder Spurenelemente und diätetische Prävention von Sekundärschäden der Erkrankung und Therapie.
